Further treatment is required after previous surgery

Kayla, who is from Malacca, suffers from biliary atresia and underwent Kasai surgery when she was about one month old. Since then, she has suffered from recurring fevers, jaundice and poor nutritional status. The University of Malaya Medical Center recommends that family members learn about liver transplantation and contact Xia Qiang’s team at Shanghai Renji Hospital.

Cross-border assessment and ethical review

The two parties communicated about the condition remotely, and the family prepared funds and procedures for coming to China. The mother is evaluated as a proposed donor, and relevant transplant arrangements undergo ethical review and other procedures. This case is a related liver transplant: the mother donates part of her liver, rather than waiting for a liver from an unknown source.

Mother and daughter underwent surgery together

The team performed laparoscopic liver harvesting for the mother and performed liver transplantation for the child. Abdominal adhesions caused by previous surgeries and small blood vessel anastomoses in young children increased the difficulty of the operation; the perioperative team took care of both the donor and the recipient at the same time.

Return to Malaysia after recovery

Reports recorded that the child recovered well about two weeks after the operation, and the family subsequently returned to Malaysia. Long-term work after transplantation also includes taking medications as prescribed, review and infection monitoring. This case illustrates the collaborative process of overseas referral, donor-recipient assessment, and the pediatric transplant team.
